Peptides for Longevity: An Evidence Map
The honest guide to what works, what’s hype, and what’s risky.
We don’t sell hype. Some peptides below are FDA-approved drugs with large randomized trials behind them; others have millions of injections in the wild and zero completed human trials. Every entry carries an evidence grade (A–D) and a legal/safety flag — that’s the whole point of this page.
How to read the grades
| Grade | Meaning |
|---|---|
| A | Multiple RCTs / approved indication. Effect real, magnitude known. |
| B | RCTs exist but limited (small, short, surrogate endpoints, or narrow population). |
| C | Mechanistic + observational + small uncontrolled human data. Promising, unproven. |
| D | Animal data only (or human anecdote). No controlled human efficacy evidence. |
Legal/safety flags: 🟢 approved/OTC · 🟡 Rx or cosmetic, regulated · 🔴 research-chemical gray market (not approved; contamination + unknown long-term risk).
The map at a glance
| Peptide | Use case | Grade | Flag | LN take |
|---|---|---|---|---|
| GLP-1 RAs (semaglutide, tirzepatide) | Weight, cardiometabolic, emerging aging | A | 🟡 Rx | The most evidence-backed “longevity” peptide story of the decade — but a prescription drug. See the GLP-1 deep-dive → |
| Collagen peptides | Skin, joints, muscle (with training) | B | 🟢 OTC | The one you can actually buy that has real (if modest) trial support. |
| GHK-Cu (copper peptide) | Skin repair, topical anti-aging | B− | 🟡 cosmetic | Best-evidenced topical peptide; dermatology-backed. |
| BPC-157 | Tissue / tendon / gut healing | D | 🔴 gray | Massive hype, ~3 tiny human pilots, zero completed RCTs. |
| TB-500 (thymosin β4) | Recovery, healing | D | 🔴 gray | Same story as BPC-157, even thinner human data. |
| Thymosin α1 | Immune modulation | B− | 🟡 Rx* | Real clinical use in some countries; not a consumer longevity supp. |
| Epitalon, MOTS-c, Humanin | “Longevity” / mitochondrial | D | 🔴 gray | Biohacker fringe; intriguing biology, essentially no controlled human evidence. |
| GH secretagogues (ipamorelin, CJC-1295) | GH/IGF-1, body composition | C/D | 🔴 gray | Raise IGF-1 — which cuts against most longevity biology. |
The four that matter most
1. GLP-1 receptor agonists — Grade A · 🟡 Rx
Semaglutide and tirzepatide are peptides, and they dominate the longevity conversation for good reason: large trials show major weight loss plus a ~20% cut in cardiovascular events, kidney protection, liver benefit, and emerging anti-inflammatory and neuroprotective signals. The evidence here is in a different universe from the injectables below — but it’s a prescription medication with real side effects and a muscle-loss caveat. Read the full GLP-1 deep-dive →
2. Collagen peptides — Grade B · 🟢 OTC (the monetizable one)
The best consumer-accessible evidence of any peptide here: RCT meta-analyses show improved skin elasticity and hydration over 8–12 weeks; moderate support for joints/tendons; and, paired with resistance training in older adults, improved body composition and strength. Modest but real, safe, cheap. Won’t transform you — but the trial support is genuine and the downside is near-zero.
3. GHK-Cu (copper tripeptide) — Grade B− · 🟡 cosmetic
The best-evidenced topical peptide: peer-reviewed dermatology supports skin remodeling and wound-repair signaling. Stick to topical, cosmetic evidence; injected GHK-Cu is a different, unproven, gray-market story.
4. BPC-157 — Grade D · 🔴 gray market (the honesty centrepiece)
One of the most-discussed peptides in biohacking, with huge anecdotal enthusiasm for tendon, gut and tissue healing. The reality as of 2026: zero completed, published randomized controlled trials in humans for any indication. The entire human literature is ~3 tiny pilots (an IV-safety study in 2 participants,1 a 12-patient interstitial-cystitis pilot, a 16-patient knee-pain series), none placebo-controlled. 2025 systematic reviews are blunt — one orthopaedic review found 35 preclinical studies and a single human study;3 a gastroenterology review called its use investigational.2 It’s not FDA-approved; it sat in FDA Category 2 (barred from compounding) until April 2026 and now awaits a July 2026 advisory review, it’s WADA-banned, and gray-market contamination is well documented. What’s new: the first randomized placebo-controlled trial is now recruiting (hamstring strain).4 Our line: “millions of injections, zero completed RCTs.” Full BPC-157 deep-dive →
Safety & legal reality (the section nobody else writes)
Gray-market injectables (BPC-157, TB-500, MOTS-c, epitalon, GH secretagogues) are sold as “research chemicals,” not medicines — no manufacturing oversight, and independent testing repeatedly finds contamination and dose inaccuracy. Long-term safety is unknown for essentially all of these. GH secretagogues raise IGF-1, which runs against much of the longevity evidence base. Even approved GLP-1s need medical supervision. We never tell anyone to source gray-market peptides — the genuine recommendation for anything 🟡/🔴 is to talk to a physician.
The deep-dives (spokes)
Each peptide gets a full evidence-graded breakdown, ordered by evidence × demand × monetization:
- Collagen peptides — the affiliate-viable, evidence-backed win.
- GLP-1 for longevity — the authority anchor (live now).
- BPC-157 — the credibility piece; tracking the first RCT.
- GHK-Cu — topical, cosmetic-affiliate-viable.
- The gray-market frontier — TB-500, GH secretagogues, MOTS-c, epitalon.
